Medication Administration Record Sheet (MARs)

We can arrange for the collection or delivery of medication

  • A laser generated MAR chart for improved clarity and safety will be provided for all medication supplied to the home, including monthly and acute items
  • The MAR charts have carer’s notes on the reverse of the MAR chart for nurses and carers to write any supporting notes, e.g. cross referencing
  • PRN medication would be included on the MAR chart without dose times to support the adherence to PRN protocols
  • Blank MAR charts will be supplied for emergency use such as new admissions or hospital discharges
  • All items dispensed including dressings, appliances and nutritional feeds would be included on the MAR chart
  • All MAR charts will contain a specific time of administration for each medication, e.g. 08:00, 16:00 etc
  • IPSA Pharmacy will agree with the Care Home a daily cut-off time for the supply of interim / acute / emergency prescriptions. This time will be no earlier than 3pm but may be later subject to local arrangements
  • This service will be supported by our strong network of 100-hour per week & late night pharmacies offering increased access for emergency dispensing and advice for evenings and weekends
  • Interim / Emergency Acute Prescriptions and Out Of Hours support are also available
  • Separate MAR charts for creams for carers use can be supplied
  • All prescriptions submitted to the pharmacy by the cut-off time will be delivered the same day
  • All items will be supplied with a printed MAR chart and Patient Information Leaflet
